I choose to play him intelligently, so he will not fight to the death, and players should absolutely try NOT to encounter him in his lair or they are probably fucked (hence the lair actions). Many of this immunities are AOE, which will cause the direct spell attacks to trigger on the ring.

I also choose to be mindful of his ring of spell turning I gave him, and tailored his spell immunities to that. You can achieve this by making sure you have a 4 - 6 spells for each level in your list and pick the appropriate ones to cast (minding spell slots!) within reason.

While this seems like cheating, the deadliness of Larloch is that he almost always is prepared which means he seems to have the particular spells ready to deal with a situation. I would recommend selecting a range of spells to make him extra deadly, and not pick a particular one as "prepared". The objective should be to not encounter him in his lair. Like you he will be THE epic-level encounter for my level 20 group in his lair. I made him intentionally overpowered to dissuade players from attacking him. I have a version that I use for my campaign. I can see from the sample on DM's Guild that he's in there- anybody seen/used Larloch's stat block and have thoughts on it, or have suggestions on other/better soucebooks stats for him? At $12 for the PDF I'm not sure I want a pre-made one that bad unless it's really cool. Looking at the sources on the Forgotten Realms Wikia pages, it looks like Lords of Darkness from 3e might be an option. I'm considering having the character make a cameo toward the end of this campaign (Rage of Demons), stealing something that was his "mission" when the player controlled him, leaving a possible epilogue where the players pursue him back to Warlock's Keep, which I'd plan to make into some crazy epic-tier encounters culminating in a battle with the lich himself.
